A Message from the Superintendent
To the Parents/Guardian:
Welcome to the Seaford School District. We are happy to have you here. Please be aware that it is a State of Delaware requirement that all children entering our schools must have had a recent physical examination. If you do not have a record of a physical examination for your child within the past twelve months, please make an appointment with your doctor or the public health clinic as soon as possible ( notify the school nurse of the appointment date and time).
It is also required that within fourteen calendar days of enrollment, you present evidence to the school that the basic series of immunizations have been initiated or completed. The following series of immunizations are required:
Ø DPT (diphtheria, pertussis, tentanus) – If your child had the four dose series as a baby, check with your family doctor or health clinic about getting a booster. (A child who received a fourth dose prior to his/her fourth birthday must have a fifth dose.) A booster dose of tentanus should be given at ten-year intervals.
Ø MMR (measles, mumps, rubella) – Two doses of measles vaccine (one dose being administered on or after 12 months of age, the second dose after the fourth birthday), one dose of rubella vaccine (administered after the age of 12 months), and one dose of mumps vaccine ( administered after the age of 12 months). The combination vaccines of measles, mumps, rubella (MMR) can be used to meet this requirement.
Ø Polio (poliomyelitis [OPV of IPV]) – Primary series of four doses (If the third dose was given on or after the child’s fourth birthday, a fourth dose is not required.)
Ø Mantoux (tuberculin skin test) – Proof of testing within the last twelve months plus documentation of results.
Ø Hepatitis B Vaccine – Three doses beginning in the 1999-2000 school year with kindergarten and grade seven students. By adding a grade at each of the levels, by the year 2004-2005, all students will be required to have the vaccine.
Ø Varicella – One dose is recommended.
Information of Lead testing is needed for children age 5 or younger.
The Shipley State Service Center is available for physicals and immunizations for those patients who have no insurance or physician. Please call 628-2006 to schedule an appointment. An open clinic for immunizations is offered at the clinic on Wednesdays from 4:00 pm to 7:00 pm for those who qualify and have no insurance or physician. No PPD’s are done on Wednesday nights.
Note: If the student does not get the series of required immunizations according to the Division of Public Health’s schedule, your child could be excluded from school.
